Notify Public Health of Your Child's Immunization Records
Just a reminder that parents need to provide information about their child's or children's immunization history directly to Public Health. This information is not provided to Public Health by the child's health care provider.

Please note that incomplete or out of date immunization records may result in your child being suspended from school, as per the Immunization of Schools Pupil's Act.

Please call 519-883-2007 Ext. 6182 or visit https://e-immunization.regionofwaterloo.ca/ to update your child's or children's immunization records by phone or online. Please remember that each time your child receives a new vaccine you need to inform Public Health.
